Medication Hesitancy Is Normal

If your first reaction to the idea of medicating your child was hesitation, that is completely understandable. Parents worry about dependency, personality changes, side effects, and whether their child might simply outgrow the issue. These are not irrational fears — they are the concerns of a parent who cares deeply.

At PBHG, we do not dismiss those concerns. We start every conversation by listening to them. Our psychiatric nurse practitioners are trained to meet families where they are, answer questions honestly, and present all options — including the option not to medicate.

When Medication Is — and Isn’t — Recommended

After a comprehensive psychiatric evaluation, our NPs may recommend medication, outpatient therapy, a combination, or a referral for further testing. Medication is one tool within a broader plan that considers your child’s full picture — home life, school performance, social interactions, and emotional wellbeing.

Our philosophy is individualized, not cookie-cutter. We balance evidence-based clinical practice with respect for your family’s values and comfort level.

How We Manage Medication for Children

The Initial Evaluation

Every medication management relationship begins with a thorough psychiatric evaluation. This first appointment is approximately 90 minutes, always in-person, and conducted in our sensory-friendly, child-first environment. We use interview-style assessments designed to be comfortable for children, not clinical interrogations.

We gather input from you, from teachers when possible, and from your child directly. For ADHD specifically, PBHG performs full diagnostic evaluations and serves as the first step in the process, referring to trusted partners for additional neuropsychological testing when needed.

Titration: Finding the Right Approach

If medication is part of your child’s plan, we start at the lowest appropriate dose and adjust gradually. This process — called titration — relies on ongoing feedback from you, your child, and their teachers. Follow-up appointments are approximately 30 minutes, and after the initial evaluation, can be conducted virtually when clinically appropriate.

We monitor progress closely and adjust as your child grows and changes. The goal is always the minimum effective approach that supports your child’s ability to function and thrive.

Non-Stimulant and Alternative Options

For families concerned specifically about stimulant medications, alternatives exist. Non-stimulant options such as atomoxetine, guanfacine, and clonidine work through different mechanisms and may be appropriate depending on your child’s diagnosis and response. Our NPs and their collaborating physician will walk you through each option in plain language.

The School Connection: Supporting IEP and 504 Plans

One of the most powerful outcomes of a psychiatric evaluation is documentation that supports your child’s success at school. Our NPs provide formal letters and clinical recommendations for 504 and IEP plans — the kind of clinical weight that school districts require before they act.
